Where insurance knowledge goes missing
An insurer's discipline lives in decisions it has already made. The exposure it declined and why. The endorsement added after a bad claim. The reasoning that made a marginal risk acceptable one year and not the next. Most of it is written down somewhere. Very little of it is findable by whoever is looking at the next submission.
So positions drift. Two underwriters reach different answers on the same exposure, both defensibly, because neither can see what the house has already decided. The cost of that turns up later, in a claim nobody priced for or a wording nobody meant to be using.
Search does not solve it. Full-text search finds documents mentioning a peril; it does not find the submission where the house took the opposite view and explained the reasoning. It also has no idea who is entitled to see which account.

What this means for insurance confidentiality
The general architecture is the same everywhere. These are the parts that matter specifically here.
- Account-level entitlement
- Underwriting files carry obligations to insureds and brokers. Access follows the account, mapped before indexing rather than discovered when somebody asks the wrong question.
- Accountability after the fact
- Which material informed a decision, and when, is logged and reviewable — which is what a conduct or reserving review actually asks you to produce.
- Personal and medical material
- Categories carrying personal or medical information are defined before anything is indexed, and can be excluded from the corpus or kept inside your own network entirely.

Questions insurers and brokers ask
The ones that come up in nearly every evaluation in this industry, answered plainly.
Does this make underwriting decisions?
No. It finds what the house has already decided and shows the reasoning with its source attached. The decision, and the signature on it, stay with the underwriter. Retrieval and drafting are bounded deliberately: the system finds and composes, and it does not decide.
Our guidelines are in bulletins going back years, and some are superseded.
Then currency is the first thing worth establishing. Answers carry the document and its date, so age is visible rather than assumed, and which bulletins are authoritative is decided with you during the build for the lines where getting it wrong would matter.
Does it work for a broker as well as a carrier?
Yes. The material differs — submissions and market correspondence rather than guidelines and declinations — but the problem is the same one. The house has taken positions before, they were reasoned, and nobody can find them when the next one comes in.
What about personal data in claims files?
Which categories are indexed at all is a decision you make, written down during architecture rather than discovered later. Material carrying personal or medical information can be excluded from the corpus, or confined to hardware inside your own network so it never reaches an external model.
